Registration Instructions
All 2012 Blue Chip Showcase sessions will be
held at Bryant University in Smithfield, Rhode Island.

Please read
the registration instructions below carefully!
There are five steps to registering:
1. Have your coach email a recommendation for your son
to
tspencer@bluechiplax.com.
The recommendation must specify "Varsity Division" or
"Elite Division" as outlined in the "Varsity & Elite
Tab" on the menu to the left. You will be notified
by mail with an invitation to register for your assigned
program.
2. When you receive your invitation letter,
go to Camp Store by clicking on the link on the
menu at the left. Select Registrations and
complete payment - obtain
transaction or receipt ID.
3. Return to this page and complete the Registration form by clicking on the
link at the right under "Registration and Medical
Forms."
4. Print the confirmation letter from the
completed payment and registration form.
5.
Print the Blue Chip Medical Form and follow the
instructions on the right hand side of this page.

If you have extenuating circumstances which
prevent you from paying online, please contact our office at

Registration and Medical Forms
Your registration is not complete and your place is
not guaranteed until payment, registration, and medical
forms are submitted.
Medical Forms MUST be submitted by Fax or by email only.

PLEASE DO NOT MAIL MEDICAL FORMS!
Medical Forms must be submitted within 7 business days of
enrollment. In an effort to keep the paperwork consistent
for our Athletic Training Staff and to assure the proper
processing of your son in the event of an emergency or
injury we are REQUIRING all campers to use the Blue Chip
Medical Form ONLY.
WE ARE NO LONGER ABLE TO ACCEPT COPIES OF SCHOOL
PHYSICALS.

Cancellation & Refund Policies
Cancellation and Refund Policies are based on
the DATE of cancellation not the REASON:
Cancellation notification must be in
writing by email to
info@bluechiplax.com
Cancellation prior to 60 days of the start of
camp, there will be a $75 processing fee for all
refunds.
If you cancel within 60 days of the
start of camp, $150 will be retained from your
camp tuition with the remaining balance refunded
in full.
Cancellation within 30 days of
the start of camp, $300 will be retained from
your camp tuition with the remaining balance
refunded in full.
Cancellation within 72 hours of the start of camp there will no longer
be any refunds processed.
We DO NOT accept day students - all registered athletes will need to stay in
the dorms on campus.
NO EXCEPTIONS!
