This letter is to certify that we have received your application for _ and your payment in full in the amount of $645 for a 2010 summer session:

Rising Seniors June 28-July 1, 2010 at Bryant University, Smithfield, RI or
Rising Juniors July 5-8, 2010 at Bryant University, Smithfield, RI or
Rising Sophomores July 26-29 at Golden Goal Tournament Park, Fort Ann, New York

 Note that you must print, complete, and submit the medical form from the link at the bottom of the page prior to your son's arrival at camp.

Registration information, what to bring to camp, directions to campus, transportation information, and the medical form can be found and downloaded on our website at www.bluechiplax.com (click on the Blue Chip 225 tab in the upper left corner).

All campers are required to submit a Medical Form with immunization record signed by their physician.  It does not need to be our specific form, school physicals that contain the same information as our form are acceptable.  Campers without the proper Medical form will not be able to participate in any field activities as mandated by the State of Rhode Island and the State of New York.

There is a $50 key/damage deposit check required (checks made payable to Blue Chip Lacrosse).  Please mail key/damage deposit check with completed medical form PRIOR to the start of camp.  When the room keys are returned at checkout and the residence passes inspection the deposit will be returned.  If the key is lost or any damages are reported the deposit will be retained.

Look for reminder notices to be sent out 30 days prior to the start of camp.  We look forward to seeing you at registration for Rising Seniors on June 28^th or for Rising Juniors July 5^th between 12-2pm at Bryant University or the registration for Rising Sophomore Camp July 26th at Golden Goal Tournament Park at Fort Ann, New York.  The specific location of registration will be announced in the reminder notice.   Thanks for your interest in Blue Chip Lacrosse!

**Cancellation and Refund Policies are based on the DATE of cancellation not the REASON:  Cancellation prior to 60 days of the start of camp, there will be a $75 processing fee for all refunds. If you cancel within 60 days of the start of camp, $150 will be retained from your camp tuition with the remaining balance refunded in full.  Cancellation within 30 days of the start of camp, $300 will be retained from your camp tuition with the remaining balance refunded in full.  Cancellation within a week of the start of camp there will no longer be any refunds processed.
